Failure Modes and Effects Analysis (FMEA) Scoring System

When using failure mode effects analysis (FMEA) to evaluate a process, team members must assign a numeric value to each failure mode rating the likelihood of occurrence, severity, and detection of failure. These numeric values are needed to calculate the Risk Priority Number (RPN) for each failure mode.
